MEMBERSHIP CANCELLATION FORM

We're sorry to see you go!

  • Hale Strength Training (HST) requires a minimum of 21 days’ written notice to cancel or downgrade any membership type. Notice must be received on or before the 21st day prior to your next scheduled billing date.

  • If the required 21-day notice is not provided, one additional full membership payment will be processed.

  • If notice is submitted after your billing date, HST allows the additional paid month to be used at any time within the following six (6) months.

  • If you choose to reactivate a membership after cancellation, the membership rates in effect at the time of reactivation will apply. Membership rates are subject to change.

  • If you are traveling or injured and would like to put your membership on hold instead of cancelling, please submit a Membership Pause Form.

  • If you have any personal equipment kept at the gym (barbell, jump rope, gear in cubbies, etc), you must pick it up during business hours within 7 days of submitting your cancellation request!
